| NISL CODE OF CONDUCT - U11 and UP TEAMS - must be signed and carried to every game with the liability release. The Signed Code of conduct will be collected during our tournament team Check-In. You must turn in a completed form or you will not be eligible for the tournament. |
| ACADEMY CODE OF CONDUCT - U09/U10 ACADEMY TEAMS - must be signed and mailed in by the first weekend of the games to NISL c/o Angel Hall, 919 E. Center Street, Warsaw, IN 46580. |
